PROFESSIONAL REFERRALS
This page is specifically for professionals who wish to make an orthodontic referral. Patients who are looking for an orthodontist in Chiswick can either come for a free, no obligation orthodontic assessment where they can discuss suitable treatment options with out treatment co-ordinator – or they can proceed directly for initial examination with a written treatment plan outlining the course of treatment and desired outcomes.
With each referral we promise to:
- Send an acknowledgement on receipt of your referral
- Treat your patients as you would wish them to be treated
- Send copies of all of our radiographs for you to keep
- Send correspondence informing you of progress at every step of your patient’s treatment
- Liaise all appropriate information concerning multi-disciplinary cases
- Provide an excellent working relationship
- Encourage your patients to see your hygienist if required
- Provide a written summary at the end of your patient’s treatment
We request that you:
- Include full details of your patient, including a contact telephone number.
- Include your name or their referring dentist’s name on all correspondence
- Encourage your patient to maintain excellent oral health throughout their treatment
- Maintain regular dental and hygiene examinations with your patient
- Carry out any extractions which may be required as part of the orthodontic treatment
- Contact us straight away if you have any queries or concerns
